Default Driven clutch wobble

I'm new to this golf cart thing and I can see how easily it becomes an addiction.

I just acquired a 1998 Ezgo TXT w/295cc Robin's engine. Did the normal tune up and discovered the the secondary clutch looked severely bent, I loosened the clutch, removed, and re installed, if I just snug the bolt, the clutch runs true, as soon as I start tightening the bolt, the clutch starts to appear bent. The bolt is not bent.
The tighter I get the bolt, the worse the "wobble". I can't seem to figure out what is going wrong. Could there be a sleeve missing?
Thanks guys for any help.

I've checked the bolt, it's straight. The centering collar is snug. Checked the shaft with a dial indicator and it's nearly perfect (within .001).

This will have a diagram of the clutch so you can see if there is something missing. http://products.jacobsen.com/img/manuals/28790-g01.pdf

I got it figured out, the splines on the secondary are so worn out that it is allowing the clutch to wobble on the shaft. Got a new used clutch and it fixed the problem. Thanks for the link and the help.
