Any chance you have watched the driven clutch (back one) to make sure you are going fully into gear when changing from forward to reverse? Just a thought it might not be shifting fully but I'm betting hub as well.

Took it all apart. It was the passenger side. There was a nice pile of black dust as I took the wheel off. I did notice that both sides castle nuts were not tight at all. So I should tighten the driver side up while im in there to the 70ft lbs right?

I would tighten it to 70 and then a little more to get the cotter pin where it will go in and you should be good to go.
