Do I only hook up positive to this? What do I use for ground

yes that is for positive only and you would need to use a separate ground strip. Or get another style with both

I am new to these fuse boxes. Does the one you showed feed both positive and negative back to my lights

you would run the voltage reducers or separate 12 volt battery leads + /- to the two center studs top and bottom of the fuse block , then run the + from lights , radio or what ever to the bottom 6 screws and - would run to the top 6 screws

I guess I should of asked if you knew voltage reducers , they come in different amperage depending on your needs . I run a 30 amp 48 to 12 volt reducer . what it does is takes the complete 48 volt pack down to 12 volts . This is the one I have (dont get scared at the price if you look around you can cut that in half)

then from the reducer to the fuse block and your good to go

This is mine (with the cover off). The cover goes over the positive side.
