DCS Forward and reverse

Hi all, I am working on a DCS cart and the owner complained that it would go into reverse when it was in forward. This happened several times, and then nothing. He had fried the controller. I replaced the controller and tightened the loose connections and it worked fine....all wires were cool to the touch after running it....no problems. 4 days later, he calls me and says that it has begun doing the same thing, going in reverse while it is in forward.....this is a new one on me.....one controller going bad is one thing....now a second...?????? seems like there is something else happening here. I have gone over all the connections from batteries to solenoid, everything appears good. Any suggestions?
