As ScottyB said, a loose connection can cause a stud or two to melt into the plastic. This isn't a worn out condition, and can occur in one day, if loose, or under-rated for the load - hills, speed, load, passengers, etc.

I tightened it down further and tightened the contact bolts (gold ones) and its running again. Should I just replace the fnr or take it out and take it apart to check the studs?

The nut should have been run all the way down or darn close to it initially. Running with poor contact can cause pitting so you should remove the center shaft and look at both sets of contacts. You can clean them up if the markings are minor.
