You have an early model year 2000 TXT PDS.

Attached .pdf files are the Service manual and Parts manual that include that cart.

When EZGO made the transition from the DCS drive to the PDS drive, apparently they just reshaped the DCS F/R hockey puck to meet the different switching needs of the PDS controller and a few months later they finally realized they could replace the complicated (and expensive) F/R assembly with a SPDT/Center-OFF toggle switch.

On they other hand, they may have had a bunch of DCS F/R assemblies in stock, so they modified and used them until they ran out, then charged to the dash mounted F/R switch.

Attached images are the PDS and DCS hockey puck type F/R switch assemblies. The look the same except tor the shape of the rotating cam and a different beeper is used.

FWIW: The DCS F/R can also be replaced with a toggle switch, but a DPDT/Center-OFF is needed.
