Contact A is not making contact with contact D.

I've never worked on the F/R switch assembly in a series cart, so I don't know what is wrong with it, but something is definitely wrong.

Hopefully, someone familiar with the mechanics of a series F/R switch will drop by. Or starting a new thread asking about what is wrong might get a quicker response.

You know...I think I will check the length of the contacts and see if one is off by just a little bit. A small gap in the contact surface would do it...

After removing the FNR, you may have an alignment issue. I always set my FNR to exactly N (neutral), then move the bracket left and right, until the handle is exactly up, before tightening the bolts. The holes are slotted for this.
