94 ezgo dcs no forward

Not seen this before. With the wheels up, reverse and forward work. On the ground, reverse is fine but no forward. Is this the motor or controller?

Can you give the Manufacture Code # from the tag in the glove box?

Don't know if it is the eyesight or poor typing skills! That should have been 2004.
