Default issue with Sevcon Milipak

I have a 08 Bad By buggie and am having an issue. When in forward, I push the accelerator past 1/4 throttle the rear motor controller faults and flashes 12 times. It is fine in reverse to full throttle.

My thought is, the motor is damaged? would you be able to go in reverse without issue, but forward would not work? I know the previous owner ran this on just the rear motor for some time and could have damaged it.

What do you guys think?

Are you sure there is not a pause in between flashes? the manual shows 10 being the maximum

I know. Nope, 12 straight through. IT works fine when in reverse and at 1/4 throttle. Very weird.
