Default Reason for zone fires ???

I see some talk of zones catching on fire / batteries exploding. What are the reasons?

Lack of maintenance ( no water in batteries)?
On board chargers cooking batteries?
Some kind of short in the cart shorting out the batteries?

Anyone know of any specifics?

When I replaced my batteries, the person who delivered the batteries had been in the business for over 30 yrs and had plenty of knowledge when it comes maintenance and care of batteries.
I showed him the post and the fire and asked him what he thought about it. He said poor maintenance certainly could have caused the fire and stressed how important it is to check the water level often!
