It was the whole fan I installed, not just the switch. I'd guess the manufacturer might be the only place to order the switch. Looks proprietary to me.

I cannot see the back side of the board posted, but assuming it has an FET soldered on the back, that is a simple 555 PWM dimmer.

Search amazon for 12v PWM dimmers, they are less than $15.
